BUTTON NOSE PET SHOP AND GROOMING RELEASE AND DISCLAIMER FORM

Your pet is important to us. Because we care about your pet’s safety and well-being, we want to assure you that every effort will be made to make your pet’s visit as pleasant as possible.   This disclaimer and release form is to protect your pet(s) and to review our grooming agreements at Button Nose Pet Shop. In the best interest of your pet, we request for your review of this form and your permission to groom your pet(s).

I warrant that my dog is current with the vaccinations required by law, and that my dog is healthy to the best of my knowledge.
If pet is hurt or becomes ill, Button Nose Pet Shop has permission to call or take pet to veterinarian of our choice if pet's vet is inaccessible; or administers medication; or gives other advisable attention, with our discretion and judgment, and such expenses will be paid promptly by owner unless injury is direct result of negligence of Button Nose Pet Shop. We will not be held responsible for any sickness or injury caused by the pet to itself during grooming. Button Nose Pet Shop is not responsible for any accidental death of pet of the nature of pre-existing health condition or natural disasters (fire, storm, flood, etc.).

Button Nose Pet Shop will not be held responsible for clipper burn and/or minor nicks resulting from grooming of matted, neglected coats or for irritation caused by removing coat from pets possessing mild to severe skin allergy, nor will we be held responsible for stressful effects grooming may have upon an elderly pet.

We reserve the right to charge additional fees for services we consider over and above the norm covered by our standard rates. We also reserve the right to refuse service to customers whose pet may pose a threat to us, to itself or to the other pets left in our care, whether it be an aggression problem, health problem, or parasite problems.

Owner agrees to be responsible for any property damages by pet(s).

Current Vaccinations/Veterinarian Information: Proof of Vaccination or current Titer testing as well as current Veterinarian information must be provided before grooming will occur. Special circumstances will be taken into consideration.

Aggressive or Dangerous Pets: Owners MUST inform Button Nose if your pet bites, has bitten, or is aggressive to people, other pets or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet, and protects both the pet and the groomer. Button Nose reserves the right to refuse/stop services for such pet(s) at any time before or during the grooming process and charge a Handling Fee in addition to the regular grooming charge ($20 min and up).

Health or Medical Problems & Senior Pets: Grooming procedures can sometimes be stressful, especially for a senior pet or pet with health problems, and can expose hidden medical problems or aggravate a current one during or after the groom. Because senior pets and pets with health problems have a greater chance of injury, these pets will be groomed for cleanliness and comfort, in styles that will not add to their stress. In the best interest of your pet, this Agreement gives Button Nose permission to obtain immediate Veterinary treatment for your pet should it be deemed necessary by staff. We will do our best to contact you first, then take your pet to your authorized Veterinarian or to our Emergency Veterinarian. All expenses for Veterinary care are the responsibility of the pet’s owner.  We have the right to refuse any service that does not fit our grooming service.

Parasites: If fleas or ticks are found on your pet during the grooming process, your pet will be treated with a Flea and Tick shampoo to kill the parasites and you will be charged an additional fee. Ticks found will be removed for an additional charge. If ticks are found, we strongly suggest you have your pet tested for Lyme disease. Please note that parasites are a health hazard to your pet as well as to humans.

No-Shows & Cancellations:  Must give 24 hr. notice prior to the appointment in order to cancel without being charged a fee.  48 hr. cancellation notice on weekend and major holidays.  No Shows, No Calls and last minute cancellations are subject to a $25.00 FEE PER PETs, which will be added to your next ticket. Pre-Payment for same will be required before another appointment is booked. We understand there may be emergency situations and will work with you, but not on a continual basis. Please be respectful of our time as we are a by- appointment business, and another client could have taken your appointment if we had known. Note: Clients 15 minutes late risk losing their appointment without notice. Clients are asked to call one day in advance for cancellation.

Hold Harmless Agreement: By signing this contract you (or your Agent) agree to hold Button Nose, its owners, operators, employees, officers and directors harmless from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown to Button Nose. It is also further understood and agreed the terms of this agreement can change at any time, without notice, and will overwrite any and all prior signed contracts or releases. It is further understood this clause applies to any and all pets groomed.

Pick-up and Drop-off: Dogs will be given an ESTIMATED time of completion upon drop-off. Please understand that this is an estimation and for many reasons beyond our control, circumstances may arise where grooming may be done early or may take longer than initially quoted. You will be called or texted when your pet is ready for pick-up. We ask that dogs are picked up within 1 hour of our call, unless you have pre-arranged additional time in our care after your grooming service. If your dog is not picked up within the hour of being called, you will be charged a $20 per hour holding fee. Due to available space we may not be able to accept clients any sooner than 15 minutes before their scheduled appointment unless it has been pre-arranged.

Photo/Social Media: The Owner/Guardian agrees and consents to video, photography, and/or reproduction of any other manner of the likeness and/or activities of the Pet(s) and allows Button Nose to make unlimited use of said reproduction: i.e. website, advertising, photography, social media, brochure, marketing materials, etc. It is fully acknowledged that there will be no compensation in any form, including monetary, of said use, now or in the future. If you do not want video or photos taken, please let associate know prior to starting your groom appointment.
